Neurophysiology

Приєднався 7 гру 2015
This channel is primarily dedicated to topics relevant to Clinical Neurophysiology, medicine's best kept secret. General and topical health issues will also be covered from time to time.
The primary aim of this channel is educational.
As the channel has grown, I have an ever increasing 'in-box' of questions. If it's a general question relating to a given topic that hasn't been asked before, feel free to post it by the relevant video and I'll try to answer. They each have their own communities and all can benefit from the Q&As which are often quite interesting.
However, if it's a personal health question - please ask your own local doctor and also please do not email me directly.

Nitrous Oxide
Переглядів 3077 місяців тому
Nitrous Oxide can cause significant nerve and spinal cord injury. Patients often find it hard to let their medical teams know that they were inhaling it prior to admission. However, it's vital this information is shared so we can deliver the right treatment quickly. In this video I want to share a few points about this, that can help differentiate it from one of the other conditions that can mi...

EEG for kids

This video is for young kids who need to have an EEG test. I've wanted to do something animated for kids for a long time, and with the advent of AI tools (this primarily being done through Adobe) it's now become a possibility. Hope this helps someone! NB Because its made for kids, comments are automatically disabled.

In this video, I'm honoured to share the personal story of an ITU survivor and his amazing wife. Danny was hospitalised in the first wave of Covid-19. Following a stormy period with his life on the precipice of existence, he made it through. Like many survivors, they had/have to grapple with many of the issues raised in the 'Post-Intensive Care Syndrome' video, some of which persist nearly thre...

Smart watches for tracking sleep - are they worthwhile?
Thanks for the talk. RLS-UK follows The Mayo Clinic Algorithm 2019, which is now regarded as best line treatment. Iron infusions are first line therapy. The new American Academy of Sleep guidance , due to be released soon, will relegate the dopamine agonists to 'end of life scenarios' where short term benefits outweigh the inevitable long term severe complications. Nearly everyone on them WILL end up suffering severely. NICE & NHS guidance are now quite outdated. RLS-UK is campaigning to get RLS included on the curriculum at medical school and during GP training and to stop dopamine agonists from being prescribed, except in end of life scenarios. There are too many elderly patients on high doses of dopamine agonists and suffering severe, unbearable, augmented RLS. GPs do not follow up after prescribing these dangerous drugs. Impulse Control Disorder affects up to 38% of patients and there have been hundreds and hundreds of successful legal cases against GPs and neurologists for failing to warn patients of the high risk of gambling, impulse spending, etc on these drugs. Time for them to be stopped and iron infusions and gabapentinoids used first line, with long half life opioids ( methadone and Buprenorphine) used for refractory RLS. The economic savings in reduced NHS appointments/ medications and increased productivity will far outweigh the cost of training GPs properly in this common disease. I do hope new treatments are researched and trialled now that the recent gene study by Dr Steven Bell, et al, has been released.
I just got a brace is it supposed to hurt when u first put it on not the brace but the elbow or arm
Hi, no it shouldn’t hurt. Have someone check if its the right type (lots of variations but they don’t have to be uber thick and bulky, even a tubigrip can often be sufficient), the right size and not too tight. Bw Simon
Thank you for the comprehensive explanation that included the lifestyle in terms habits in bed and in sitting
This is passed down through your DNA from your mother or your father, We know this... Why would people who have this horrible thing going on and their bodies have children? What in God's name are adult people thinking when they know they're passing this on to their children to suffer every day of their life.? It's selfish, Nor socistic, and unforgivable. I have something similar, Is my parent knew this, They did not have to have children and pass this c*** on down for them to live miserable lives. That's my opinion...
Doctor my body shakes with heart beat, second when I relax my muscles sometimes starts twitching. And I feel some continuous sound in my head in silence 😢😢😢 . Please help me.
THanks!
My father has this and only showed symptoms in his early 40’s. I am now 31 - what are the chances of me having this? Thanks
Hi Jon, It's probably best to have yourself screened genetically. Ask your dad to get in contact with his Neurologist to advise further. Bw Simon
I have been suffering for years with what I now think is what I just watched. Been to every doctor, who I've been told is the "Best" including the over rated JHU circus. What I have realized is every time I go to another "Specialist" they read the notes from the prior "Specialist", tell me to push, pull and squeeze their fingers. No one looks at my skin, atrophy etc. This doctor who made this video has provided answers to every question I have without asking. I'm sorry he's not in our country, which is too busy letting politicians destroy it and get rich as we are the cattle and sheep make to shut up and get in line. Thank you for the video
Did he say 4 or 5 yes
my eyes twitch SO BAD like everyday

I was told Intravenous IV-IG can reduce Small Fiber Neuropathy via regular Intravenous administration. Recently I was introduced to a oral capsules by METACURE. The Revive Alpha Lipoic Complex with Curcumin is much faster than Intravenous IV-IG and ALA. The SEVERE NERVE PAIN, TINGLING AND BURNING are reduced about 90% to 100% in one to two weeks. The NUMBNESS is slowly going down though takes much longer.

Nice dear dr
I seem to have just gone past the first stage, following a slapped cheek infection. After two trips to A&E (the second involved giving me morphine, which only mildly dulled the pain, but at least let me sleep for 2 nights), the fourth consultant came up with this. I still have to see someone face to face about it, but the symptoms and painlevels ring true. When the shoulderpain started, I noticed that the soft tissue under the collarbone was extremely sore. Is this a usual presentation of this ? The worst for me was the lack of sleep...one can't function on 1 hour of sleep and my liver is ringing warning bells from the cocktail of pain killers. I just hope that the referral appointment comes through soon.
Like you muscles ack innocent
On set June 28th 2019 . 5 years, and the pain and weakness still haunt me . mine was brought on by stress to right shoulder went back to work part time 11 months after onset I turn 65 in September and looks like I'm done working my pain is reduced by 3/4 I never took pain meds at any time but the worst pain I lived alone so no one seen what I went through the same day work comp turned me down I recieved divorce papers . To put it lightly it's change my life for my duration here on earth looking forward to the healing when I'm done here !😊 in this life in his time 🙏👍
Do the muscles lost or wasted regain after the operation uccured of the ulnar nerve injury
Hi, it very much depends on how much wastage occurred, over what period of time, general health factors and age (younger the better). Bw Simon
Great information thank you very much
This umbrella of diseases are very cunning and sometimes move really slowly , please make sure you monitor your CK levels - specifically CK-MM which indicates skeletal muscle damage , there are three types of ck ‘s . Monitor aldolase levels and all the other typical inflammatory markers . Some variations of this despises cause a lots of pain and weakness , make sure you find providers that understand it will take a village to take care and monitor this conditions, multidisciplinary approach if you could find it . Keep moving , do not stop moving . Reach out to the Myositis Association/ very helpful. Sometimes there’s overlapping diseases , they don’t come alone ( SAD) . Can happen to anyone from children to elderly, don’t give up asking to be identified, sometimes is a long journey. Ask questions , send messages , advocate for yourself. Keep track , collect data , identify flare ups and get tested during this windows of time . Take care
Your solution to salvation is to bloody vacuum from your legs, use black mustard seeds and magnesium regularly. I recovered after 8 years and I am very well now.
Is it a problem if you have barely slept the night before the test?
Hi, not at all. We sometimes encourage sleep deprivation as it can add certain information, and it's helpful if we capture drowsiness and sleep. Bw Simon

What a surprise! Wonderful!
How to straighten a curvy base, please? I have a green and a red electrode I place on the patient along with a concentric needle
Dear Darel, great question and hope this helps. Bw Simon ua-cam.com/video/oTl24pPRA-w/v-deo.html
Thank u. That’s helped me a lot in my research for CMT
Hi Dr. I've watching your Video from first day. Thank you for your teaching. I am been a registered nerve conduction technologist quite a while, 10+ years. I have a question for F wave. Among of other parameters like latency, persistence, chronodispersion ....is AMPLITUDE of F-wave important? Thanks
Hi, a very interesting question and basically, it's not particularly utilised in routine clinical practice. There are a couple of papers regarding this in ALS but it's not really translated into something routine. There's much more interest in TMS related studies such as SICI looking for UMN dysfunction. Bw! Simon
I undergone operation in my chest and surgeon said, he to be touched ulmar nerv during surgery Now I have big pain in my hand. Can I contact you
Why on earth would someone have all that junk in the house???? We have NOTHING junkie in the house. That helps a lot!
Thanks doc, good video. I have had my right index finger/thumb twitching in exactly the same way as the clip in your video for a few weeks now, and have been getting a bit worried about it. Thanks for posting this and explaining things so well.
Believe it or not , but a kosher dill picked Sperry before bed will eliminate restless legs and arms and make you really sleep...
There is something about Pickle juice (any type) that helps with muscle cramps. Not clear as to how or why but it has been studied. Bw Simon
Hello! I learned about a new drug from ARTHEx Biotech that can greatly help people who have myotonic dystrophy. Do you have any predictions as to when this new drug will be available for purchase?
Hi, I don't know and can't guess but wish the company and all involved in every success. Bw Simon
I'm diagnosed with "atypical" myositis because apart from muscle pain, tightness and inflammation, my symptoms do not fit nearly into any of the specific named conditions. My blood work doesn't show high levels for any of the specific auto antibodies in this video. It does show a high level of anti-ku auto antibodies.
delvoloped all this symptoms after vx modern@ , Drs wont diagnose correctly.
